Change Management Solution

Is there a template or example of how to use SharePoint 2010 for change management ticketing?

I was able to create a simple form from an example I found on the internet but I am sure that someone would have thought of this and created a template for SharePoint on MSDN.  I just was not able to find one. Can someone assist with this? This would be a great addition to our site.
