I'm using the OOTB Approval Workflow with a document library. I need to ensure that we keep a log of who approves or rejects and why. The workflow keeps track of this but as the tasks get deleted and the workflow re-runs for another approval this information gets deleted. Hence I have to rely on the version history to see who approves and also column comments to see whats changed.
However, viewing version history, Modified By has the correct name but sometimes uses 'System Account'. Is there a reason why this is happening?
I need to ensure we know who approves especially when we have multiple approvers.
