Same URL, two different pictures

Why do I get two different graphics from the same URL?

I get on system 1 and my site has a yellow theme and an old set of pictures.

I get on another system and I get the current site theme colors (red) and the new picture.

Versioning is not turned on.

All of the servers have been rebooted.

Yes, I’ve cleared browser cache and rebooted workstations.

I go back to the site hours later and now I’m getting the opposite.

I’m on a workstation with the new color scheme theme and new graphics, I set up a terminal server session and click on the link for the graphic and paste it into the browser window and get the old graphic.

So I go back to the library and the Preview is correct but when I click on the picture I get the older version. I click on manage copies and nothing is listed.

I check the site and there are no ‘variations’.

Stats:

4 server farm, build version  14.0.6115.5000

2 WFEs, 2 App servers, SQL cluster backup-end (SQL 2008)

No errors showing up on any of the servers, in Event logs or ULS

Web App has 4 zones, default uses Windows Authentication - negotiated and FBA

Am I loosing my mind?
