Workflow intermittently does not start

I'm working with a workflow running on a form library, set to run for new items.  It sends an email to 2 people that contains information from the form.  There are no conditions - the email is sent for every new form.

Occasionally the workflow will not start and emails will not be sent.  

Please consider the following points:

  • The workflow is definitely configured to start for new items.  It cannot be triggered manually.
  • I've verified that the form is getting a unique filename - at first I suspected it was a duplicate filename in which case the form would be treated as an update to an existing form.
  • There is a well known scenario where workflows created with the system account don't start.  This is not my issue because  a) this workflow was not created using the system account and b) the workflow starts the majority of the time.  
  • KB 2620900 at http://support.microsoft.com/kb/2620900 documents a known issue where the workflow starts before fields have synchronized.  This doesn't affect my scenario since there are no conditions.  And because we've had this type of problem in the past, all workflows begin with a 1 minute pause.
  • I've been able to view the sharepoint logs for one of these cases, and haven't found any log messages that would indicate there was a problem.  
  • I don't think this is an email issue since the workflow is not starting.
