Get information from User

Hi All,

I have a requirement where I have to get few details from Users when the User click ( Yes ) and submit the form in a list item. Through my efforts I came to know about the "Get User information " option in Workflow setup in SharePoint Designer which sends mail to the User asking for the information. But I need it to be displayed it in webpage itself on form submit. Kindly help.

Note  - I am just an Owner in the site. I don't have  options to develop something and deploy.
